The form "State Background Information Request Form" has been renamed and revised. The new name is: Maryland Pre-Offer Check Database Form.
The Department of Human Resources is required to verify eligibility for rehire of any former State of Maryland or USM employee. All selected candidates are checked in the Maryland Pre-Offer database regardless of whether they indicated prior State or USM service. This applies to all faculty and staff positions including regular, grant funded, Contingent II and Contingent I.
The new name better reflects the purpose of the form and will eliminate confusion between the Pre-offer check and a criminal background check.
Please use the new version when submitting any hiring paperwork for approval. The form is located on the Human Resources website on the Forms page under the heading Employment. See direct link below.
